When you have been reported as absent due to a sickness, you will be required to complete a back to work form in order to tell the software that you are back in work on a particular date. Completing the Self-Certification form will end the sickness absence and send the answers to your Line Manager for review.
Instructions to complete a Self-Certification form
- Step 1: Log in to the APHR software
- Step 2: Select the red banner from your dashboard
- Step 3: Complete all mandatory fields
- Step 4: Select acknowledge to submit your form
Step 1: Log in to Activ People HR
- When returning from an unplanned absence, you'll need to log inA to APHR to let your manager know that you are fit and ready to work.
Step 2:Select the red banner from your dashboard
- A new dialog will pop up containing a small list of questions
Step 3: Complete the Self-Certification questions
- Add as much detail as possible to form to let your Line Manager know the full extent of your sickness.
- Ensure that you select the appropriate answers if your absence is a work-related accident or injury
